Project Details
The aim of the project is to provide a digital platform for the customers, providers (doctors) and case managers to address the chronic health conditions of a patient through a seamless digital application. The project involves in elevating pain points in 3 way interactions with the above mentioned audience, preparing clickable prototypes for user testing purposes. The Project was implemented in a multi-platform model across web interfaces and native mobile application. The Prototypes designed are in accordance to responsive grid systems, WCAG and ADA accessibility guidelines.
My Role
I was the lead designer and researcher for this project. This project was essentially a discovery to delivery project since there wasn't an existing digital solution i had planned to carry out an initial research followed by some clickable prototypes. I had carried out second phase of research and finally delivered the project accommodating the feedback. I had worked closely with Product Managers, Strategists, Agile IT Development team to successfully ship the project.

Impact
Concept invitation was well received and indicated a growth in user engagement.
Customers loved the idea of case management portal since they would be engaged with a dedicated case manager to tackle their chronic health condition.
Customers, Providers (Doctors) and Case Managers liked the idea of having a consolidated centralized portal through which all of the parties have a 24/7 access to the health history of customers.
The KPIs tied to these project indicated a significant growth in engagement and ROI once it has been deployed to the customers.
